About This Opportunity
Are you a fully qualified physiotherapist looking to make a difference in the community? We have an exciting opportunity for a physiotherapist to join our team at Somerset NHS Foundation Trust, working as part of our D2A service.
Main Responsibilities
As a physiotherapist with our D2A service, you will play a vital role in supporting patients back to or within their own homes. You will be responsible for carrying out specific therapy assessments, including goal setting and treatment planning, assessing risk, promoting and enabling independent functioning.
We are looking for someone who is committed to a patient-centred approach, able to prioritise and work flexibly, and has experience of working with frail and elderly populations and providing rehabilitation.
Key Skills and Qualifications
To be successful in this role, you will need to be HCPC registered and have extensive experience in community and/or acute physiotherapy. You will also need to have supervision and appraisal experience, as well as evidence of advanced clinical reasoning.
