About the Job

The Cheshire and Wirral Partnership NHS Foundation Trust is seeking a skilled Nurse Coordinator to join their team in Ellesmere Port. This non-patient facing role will involve effectively managing referrals from various care settings, utilizing different care pathways to ensure patients are referred to the most suitable service for treatment.

As a key member of the team, you will be responsible for facilitating the management of patients with unplanned care needs, adopting a patient-centred approach. This includes admission to hospital where necessary, preventing unnecessary hospital admissions by utilizing community services, and ensuring a smooth transition of care between primary, secondary, and community care settings.

The ideal candidate will possess excellent communication skills, both verbal and written, as well as extensive current clinical knowledge and evidence of ongoing professional development.

With leadership and management skills, you will have experience working in a community or primary care setting and be registered as a Registered Nurse Adult. IT literacy with good keyboard skills is also essential.
