Key Responsibilities
  • To work as part of a multidisciplinary team to assess and provide support to adults with complex health needs.
  • To develop and implement care plans that meet the individual's needs and preferences.

The ideal candidate will have strong communication and interpersonal skills, with the ability to build effective relationships with service users, families, and colleagues.

Requirements
  • A relevant professional qualification (e.g. Nursing, Physiotherapy, Occupational Therapy).
  • Significant post-qualification experience in adult health and care assessment.
  • An understanding of current legislation and guidelines related to adult health and social care.
