About the Role

We are seeking a highly skilled and experienced Social Worker to join our team at Active Care Group. As a Social Worker - Complex Care Specialist, you will play a vital role in providing high-quality, individualized care to patients with complex needs.

Key Responsibilities
  • Manage a defined caseload of patients, ensuring their needs are met and their rights are protected.
  • Develop and deliver evidence-based packages of care, including psychosocial interventions where appropriate.
  • Work in partnership with patients, carers, and other team members to achieve positive outcomes.
  • Liaise closely with other agencies to ensure a smooth transition of care.
  • Undertake care planning, maintaining contemporaneous health and social care records.

Person Specification

We are looking for a Social Worker with:

  • Professional Qualification (Degree or equivalent) in Social Work.
  • Professional Registration.
  • Experience of working with patients with complex needs, including those with acquired or traumatic brain injuries.
  • Knowledge of the Mental Capacity Act and Deprivation of Liberty Safeguards.
