Job Overview

We are seeking a highly skilled and experienced Cardiac Rhythm Management professional to join our team at Buckinghamshire Healthcare NHS Trust. As a key member of our Rapid Response & Intermediate Care service, you will play a vital role in providing expert interventions and rehabilitation to patients in need.

Your Mission:

  • To provide comprehensive specialist assessment, review, and evaluation of patient needs and develop personalized care plans to promote health gain and maximize independence.
  • To work collaboratively with a multidisciplinary team, including Physiotherapists, Occupational Therapists, Nurses, and other healthcare professionals to ensure seamless care delivery.
  • To allocate workloads effectively and manage complex cases, ensuring timely and high-quality care for patients.
  • To demonstrate professional skills and competence in leadership, care planning, and care delivery, with a focus on patient-centered care and outcomes.

Requirements:

  • Registered Allied Healthcare Professional with HCPC registration.
  • Valid and current driving license and car to travel to locations throughout Buckinghamshire.
  • Experience of working in a community or in-patient setting, with a minimum of 2 years post-registration experience.
  • Ability to develop and maintain partnership working, with a focus on patient-centered care and outcomes.
