Job Summary
Apite is seeking a skilled Health Educator to join our team in providing comprehensive health education to patients within the Northern Arizona VA Healthcare System.
This part-time role entails conducting group and individual sessions aimed at empowering patients to make informed decisions about their health and adopt healthier lifestyles.
Key Responsibilities
- Conduct group patient health education sessions as scheduled, totaling 52 events per year, across any of the five clinics within the VA Health Clinic network.
- Provide individualized patient education tailored to specific health needs and conditions.
- Cover a range of health education topics, including tobacco cessation, alcohol/substance abuse education, mental health conditions, diabetes management, nutrition, lifestyle choices, hypertension management, cardiovascular wellness, and women's health screenings.
- Empower patients to take ownership of their health by equipping them with knowledge and resources to make informed decisions.
- Collaborate with other healthcare professionals, including physicians, nurses, and allied health staff, to ensure coordinated care and support for patients.
- Maintain accurate documentation of patient education activities and outcomes in accordance with clinic protocols and regulatory requirements.
Requirements
- Bachelor's degree in Nursing (BSN) from an accredited institution.
- Current, unrestricted license to practice as a Registered Nurse (RN) in the state of Arizona.
- Minimum of 5 years of clinical experience as an RN, preferably in a primary care or community health setting.
- Strong knowledge of various health education topics, including tobacco cessation, substance abuse, mental health conditions, diabetes management, nutrition, lifestyle choices, hypertension management, cardiovascular wellness, and women's health screenings.
- Excellent communication and interpersonal skills, with the ability to effectively engage and educate patients from diverse backgrounds.
- Demonstrated ability to work independently and collaboratively within a multidisciplinary healthcare team.
- Proficiency in documentation and record-keeping, with attention to detail and accuracy.
