The successful candidate will be responsible for assessing patients on the reablement unit, including mobility, functional assessments, and cognitive assessments. This position involves discharge planning, including liaison with relatives and external agencies, as well as completing timely referrals to appropriate teams to facilitate discharges. **Essential Qualifications and Experience**

The ideal candidate will possess current relevant professional registration (HCPC Registration) and a minimum of 2 years post-qualification experience in Occupational Therapy or Physiotherapy. They will also require Enhanced DBS clearance and experience of working within the NHS.

**Estimated Salary:** £35,000 - £45,000 per annum
