Job Title: Preparing for Adulthood Social Worker

We are seeking a skilled and dedicated Social Worker to join our Preparing for Adulthood team at Somerset County Council. As a Social Worker, you will play a vital role in supporting individuals with learning disabilities and their families to achieve their goals and live fulfilling lives.

Key Responsibilities:
  • Progress cases of service users from Children Services to Adult Services
  • Work jointly with Special Education Needs Co-ordinators on funding matters
  • Collaborate with Health services within the integrated Community Team for People with Learning Disabilities
  • Attend meetings and set up meetings to draw holistic support plans with stakeholders
  • Raise and manage safeguards, and address relevant matters
  • Present cases at the weekly Risk Panel to request funding for new cases or amended care packages
Requirements:
  • Ability to work closely with families and service users, promoting their views and wellbeing
  • Fluency in presenting cases and being thorough in all interventions
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize tasks effectively
