We are seeking a skilled and dedicated Clinical Care Provider to join our team on a temporary basis. As a Clinical Care Provider, you will be responsible for providing high-quality care to patients in a general practice setting, focusing on delivering patient-centered care and improving patient outcomes.

About the Role

This is an exciting opportunity for a nurse who is passionate about delivering exceptional patient care in a dynamic team environment. You will have the chance to work closely with our experienced healthcare professionals to provide comprehensive care to patients.

Key Responsibilities

  • Effectively manage chronic disease clinics to ensure optimal patient outcomes and continuous quality improvement
  • Administer vaccinations and immunisations as required, adhering to strict guidelines and protocols
  • Collaborate with the healthcare team to deliver high-quality patient care, focusing on patient-centered approaches and evidence-based practice
  • Assist with wound care under clinical supervision, ensuring accurate treatment and progress monitoring

Requirements

  • Hold valid NMC registration and maintain up-to-date professional development
  • Have experience using the SystmOne system, with proficiency in managing electronic health records
  • Possess strong clinical skills and experience in general practice nursing, with a focus on patient-centered care and evidence-based practice
  • Demonstrate excellent communication skills and a commitment to high-quality patient care, prioritizing patient needs and preferences
