About the Role

We are seeking an enthusiastic and experienced Occupational Therapist to join our team at Epsom and St Helier University Hospitals NHS Trust. As a Band 6 Occupational Therapist, you will be part of the Sutton Health and Care at Home service, working closely with a multidisciplinary team to provide high-quality care to patients in their homes.

Job Responsibilities

The successful candidate will be responsible for:

  • Providing occupational therapy services to patients in their homes, focusing on prevention of admission, accelerated discharge, and supported discharge through bed and home-based intermediate care.
  • Working collaboratively with the multidisciplinary team to develop and implement individualized care plans.
  • Conducting assessments, goal setting, and interventions to promote patient independence and well-being.
  • Collaborating with support workers to ensure effective delivery of care.
  • Participating in research and quality improvement initiatives to enhance service delivery.
Requirements

To be successful in this role, you will need:

  • A Diploma or Degree in Occupational Therapy.
  • Relevant postgraduate qualifications and HCPC registration.
  • Significant relevant post-registration experience, including experience of working with support workers and conducting clinical teaching.
  • Effective written and spoken communication skills, with the ability to demonstrate ongoing CPD and evidence-based practice.
