About the Role: We are seeking a skilled Clinical Team Lead to join our team at Elara Caring. This role is ideal for a compassionate and organized individual who can manage patient care services and oversee staff operations.

Salary: $90,000 - $120,000 per year. The estimated salary range is based on industry standards and takes into account the level of experience required for the position.

About the Job Description:

Job Responsibilities:
  • Plan and Direct Patient Care Services: As a Clinical Team Lead, you will be responsible for planning and directing patient care services to provide diagnosis-specific care. This includes coordinating services with providers, vendors, or facilities, ensuring timely start of patient care within regulatory guidelines.
  • Oversee Staff Operations: You will oversee all patient care services and personnel, ensuring that care is delivered efficiently and effectively.
  • Process and Coordinate Physician Orders: You will process and coordinate physician orders, staff assignments, and communications between direct care team members, patients, and their families.
  • Evaluate Plan of Care: You will review and approve the plan of care and evaluate proposed changes for clinical appropriateness.
Requirements:
  • Nursing-Related Degree: An Associate's degree in a nursing-related field is required.
  • Home Care Experience: At least 2 years of home care experience as a Registered Nurse in Home Health or Behavioral Health is necessary.
  • Supervisory Experience: One year of supervisory experience is also required.
  • Licensure: A current state license as a Registered Nurse is essential.
  • Technical Skills: Proficiency in enterprise applications such as Workday and HomeCare HomeBase, as well as MS Office Suite (Word, Excel, Outlook), is required.
