Job Summary

An exciting opportunity has arisen to join Voyager Family Health as a PCN Manager, leading the development and coordination of primary care services in Farnborough. This role will play a key part in progressing the Farnborough PCN development plan, working closely with member practices and stakeholders to deliver business, data submission, transformation, and improvement initiatives.

Main Responsibilities
  1. Provide strategic transformation support and leadership to member practices, working to break down challenges to implementation across the population.
  2. Develop strong relationships and engage with a wide range of stakeholders to ensure collaborative approaches to service development and delivery, and to improve integration with the broader community services as the network matures.
  3. Manage agreed projects, focusing on developing partnerships across primary care, community services, social care, mental health, the voluntary sector, and secondary care that enable ongoing MDT development.
  4. Disseminate new information across member practices and coordinate shared learning events, actively supporting integrated ways of working while promoting standardisation for efficiency purposes.
  5. Secure the PCN's resilience by actively seeking development opportunities and coordinating proposals.
  6. Recruit and integrate new roles into primary care teams, acting as line management for these new staff.
About Us

Farnborough Primary Care Network is a group of 6 GP Practices working together with a range of local providers to offer personalised and coordinated health and social care to our 65,000-patient population. Based in an excellent location on the Hampshire/Surrey border in Farnborough, our MDT has a variety of clinical and non-clinical staff who work closely together to provide high-quality care and deliver better health outcomes for our patients.

Person Specification

Essential

  • Strategic understanding and experience of working in the NHS with extensive knowledge and understanding of the roles of the NHS organisation and of the primary care sector.
  • Good practical and conceptual knowledge of healthcare improvement methods and practices.
  • Experience of managing accounting procedures, including budget and cash flow forecasting.
  • Experience of performance management, including appraisal writing, staff development, and disciplinary procedures.
  • Experience of workforce planning, forecasting, and development.
  • Experience of working at a senior level in a project and/or transformation and/or business management area.

Desirable

  • Understanding of the governmental policy direction for primary, community, and secondary care and as evidenced in the Five Year Forward View.
  • Experience of developing primary care provision at scale, for example, working with a federation or within a collaboration of practices.
  • Knowledge of the regulatory and contractual frameworks for contracts within primary care, for example, enhanced services QOF and the standard GP contract.
  • Experience of successfully developing and implementing projects, including establishing working relationships with PCN stakeholders.
  • Experience of identifying and interpreting governmental policy.
