About the Job

This is an exciting opportunity for a dedicated and motivated professional to work as a Home Healthcare Professional, delivering high-quality care to patients in their own homes or community settings. The successful candidate will be responsible for providing a range of nursing interventions, including wound care, medication administration, catheter care, and end-of-life care.

Key Accountabilities
  • Clinical Practice: Deliver high-quality, patient-centred care, adhering to local and national policies, guidelines, and regulations.
  • Care Planning: Develop and implement individualised care plans, taking into account patients' physical, emotional, and social needs.
  • Caseload Management: Manage a daily caseload, prioritising tasks and responding to changes in patients conditions.
  • Team Collaboration: Work closely with other healthcare professionals, ensuring coordinated care and effective communication.
Requirements

To be considered for this role, you must have:

  • A Registered Nurse (Adult) qualification with current NMC registration.
  • Clinical experience in a relevant setting, such as community, primary care, or acute services.
  • The ability to work independently and manage a caseload in a community environment.
  • Strong clinical assessment and decision-making skills.
  • Excellent communication, interpersonal, and organisational skills.
