We are seeking a highly skilled and experienced Nurse Assessor/Case Manager to join our team at NHS South Yorkshire Integrated Care Board. As a key member of our Continuing Healthcare (CHC) team, you will play a vital role in delivering high-quality patient care and ensuring that individuals receive the support they need to live independently.
Key Responsibilities- Conduct thorough assessments of patients' care needs, ensuring that their requirements are accurately identified and met.
- Develop and implement personalized care plans, working closely with patients, families, and healthcare professionals to ensure seamless care coordination.
- Provide expert advice and guidance to colleagues, patients, and families on CHC-related matters, ensuring that everyone is informed and empowered to make informed decisions.
- Collaborate with multidisciplinary teams to ensure that patients receive comprehensive and coordinated care, addressing their physical, emotional, and social needs.
- Monitor and evaluate patient outcomes, identifying areas for improvement and implementing changes to enhance care quality and patient satisfaction.
- Develop and maintain effective relationships with healthcare providers, community organizations, and other stakeholders to ensure that patients receive the support they need.
- Stay up-to-date with the latest CHC policies, procedures, and best practices, applying this knowledge to improve patient care and outcomes.
- Registered Nurse (Children, Mental Health, Learning Disability) qualification.
- Experience in case management, preferably in a public sector setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Strong analytical and problem-solving skills, with the ability to interpret complex information and make informed decisions.
- Ability to work independently and as part of a team, with a strong commitment to delivering high-quality patient care.
- Proficient in using Office 365 packages, particularly Word, Outlook, and PowerPoint.
