About the Role

We are seeking a highly skilled and experienced Nurse Assessor/Case Manager to join our team at NHS South Yorkshire Integrated Care Board. As a key member of our Continuing Healthcare (CHC) team, you will play a vital role in delivering high-quality patient care and ensuring that individuals receive the support they need to live independently.

Key Responsibilities
  • Conduct thorough assessments of patients' care needs, ensuring that their requirements are accurately identified and met.
  • Develop and implement personalized care plans, working closely with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Provide expert advice and guidance to colleagues, patients, and families on CHC-related matters, ensuring that everyone is informed and empowered to make informed decisions.
  • Collaborate with multidisciplinary teams to ensure that patients receive comprehensive and coordinated care, addressing their physical, emotional, and social needs.
  • Monitor and evaluate patient outcomes, identifying areas for improvement and implementing changes to enhance care quality and patient satisfaction.
  • Develop and maintain effective relationships with healthcare providers, community organizations, and other stakeholders to ensure that patients receive the support they need.
  • Stay up-to-date with the latest CHC policies, procedures, and best practices, applying this knowledge to improve patient care and outcomes.
Requirements
  • Registered Nurse (Children, Mental Health, Learning Disability) qualification.
  • Experience in case management, preferably in a public sector setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Strong analytical and problem-solving skills, with the ability to interpret complex information and make informed decisions.
  • Ability to work independently and as part of a team, with a strong commitment to delivering high-quality patient care.
  • Proficient in using Office 365 packages, particularly Word, Outlook, and PowerPoint.
