Main Responsibilities:

  • Undertake initial assessments and develop care plans in collaboration with service users, families, and healthcare professionals.
  • Attend regular multidisciplinary team meetings to discuss case progress and develop effective treatment plans.
  • Manage a caseload of clients, providing face-to-face support and interventions in the community setting.
  • Work closely with internal and external stakeholders to ensure seamless care coordination and service delivery.

Requirements:

  • Registered Mental Health Nurse with relevant qualifications and experience.
  • Excellent communication, assessment, and care planning skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Good knowledge of mental health practice, policies, and guidelines.
