We are seeking a qualified Family Support Specialist to join our team at Wheeler Clinic Inc. in Bristol, CT.

">About the Role

This is a full-time position that involves providing direct services and case management to individuals and families in need of support. The ideal candidate will have a strong background in psychology, social work, or a related field, and excellent communication skills.

Job Summary:

The Family Support Specialist will provide front-line crisis support to individuals and families in the program by phone, in-home, and/or in the community as needed. They will also develop and implement case management plans, provide group and individual counseling, and maintain records of counseling and case management activities.

Key Responsibilities:

  • Develops case management plans and provides direct case management services to assigned clients and families.
  • Provides front-line crisis support to individuals and families in the program by phone, in-home, and/or in the community as needed.
  • Maintains records of counseling, coordination of care, child and family team meetings, and case management activities.
  • Supports daily program-specific operational functions as required.
  • Participates in the development, implementation, and facilitation of initial treatment plans.

Requirements:

  • Bachelor's degree in Psychology, Social Work, Marriage and Family Therapy, Counseling, or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to safely operate a vehicle in the community based on program needs.
