Care Manager Role
Care in the Home division of Active Care Group is experiencing significant growth, driven by its commitment to delivering high-quality care services to clients. As a Clinical Manager, you will play a crucial role in shaping the future of our division, working closely with our exceptional support workers and clinicians to ensure the best possible outcomes for our clients.
This is an exciting opportunity for an experienced clinical professional to join our team and contribute to the continued success of Care in the Home. If you are passionate about providing person-centred care and have excellent leadership skills, we would love to hear from you.
About You
We are looking for a highly motivated and skilled clinical professional with experience in leading teams and developing complex care plans. You will have a strong background in nursing and be registered with the NMC. Your excellent communication and interpersonal skills will enable you to build strong relationships with clients, carers, and healthcare professionals. You will also have a full driving licence and access to your own vehicle.
In return for your expertise and dedication, we offer a competitive salary of £45,000-£60,000 per annum, depending on experience, as well as a range of benefits including 25 days' annual leave, a birthday off, and access to our Employee Assistance Programme. We are committed to supporting the development of our staff and offer opportunities for training and professional growth.
What's Involved
Your primary responsibility will be to oversee the delivery of high-quality care services to clients, ensuring that their needs are met and exceeded. This will involve working closely with our support workers and clinicians to develop and implement effective care plans, monitoring client progress, and making adjustments as needed. You will also be responsible for delegating approved clinical skills to support workers and conducting regular observations to ensure compliance with company protocols.
You will work closely with our Care Manager team to ensure that all client records are accurate and up-to-date, and that all care plans are compliant with Active Care Group's policies and procedures. You will also be responsible for identifying and mitigating risks within care packages, working closely with scheduling teams and care managers to ensure seamless care delivery.
Benefits
We offer a comprehensive package of benefits, including a competitive salary, generous annual leave, and access to our Employee Assistance Programme. You will also have the opportunity to participate in our active learning hub, which offers a range of e-learning and face-to-face training opportunities. Additionally, you will have access to a range of discounts and savings through our Active Reward App, and be eligible for our Refer a Friend Scheme.
