Community Care Worker

We are seeking a Community Care Worker to join our Louth Crisis Home Treatment Team. The ideal candidate will have previous experience working with people with mental health problems in a nursing assistant grade or equivalent within a health and social care setting.
The successful candidate will be responsible for supporting individuals in their own homes, promoting equality for all patients and understanding their health needs. Strong communication and interpersonal skills are essential for this role, as well as a patient and empathic approach.
The role requires flexibility across a 7-day-a-week service, including weekends and evenings. You will receive regular supervision and support from the team.
As a Community Care Worker, you will have the opportunity to make a positive impact on the lives of our service users and contribute to a multidisciplinary team that prioritizes patient-centered care.
