We are seeking a qualified Community Care Practitioner to join our team in the North Ealing area. As a key member of our community care team, you will be responsible for providing mental health services to individuals in the local community. Your primary focus will be on delivering joined-up care in partnership with General Practitioners (GPs) and working closely with the Ealing North MINT team to address health inequalities in the local community.
The successful candidate will have extensive experience in mental health care, with a strong understanding of recovery approaches and psychosocial interventions. You will be an expert in counseling skills, cognitive behavioral therapy, and other relevant therapeutic techniques. Your excellent communication and interpersonal skills will enable you to work effectively with patients, families, carers, and other healthcare professionals to deliver holistic care and support.
We are looking for a highly motivated and flexible individual who can work independently and as part of a multidisciplinary team. You will be responsible for managing complex caseloads, assessing patient needs, and developing personalized care plans. Your ability to work under pressure, maintain intense concentration, and manage stressful situations will be essential in this role.
