Job Summary

We are seeking a highly skilled and dedicated Nurse Practitioner to join our team at Bedfordshire Hospitals NHS Foundation Trust. As a key member of our Hospital at Home Service, you will play a vital role in delivering high-quality, patient-focused care to adults in their own homes.

About the Role

The successful candidate will be responsible for assessing care needs, developing and implementing management plans, and providing education and training to patients, carers, and multidisciplinary teams. You will work closely with our community and hospital teams to facilitate integrated care and enable early supported discharge, preventing unnecessary hospital readmissions.

Key Responsibilities
  • Assess patient care needs and develop individualized management plans
  • Provide education and training to patients, carers, and multidisciplinary teams
  • Work collaboratively with community and hospital teams to facilitate integrated care
  • Monitor and maintain clinical standards and quality care
  • Participate in the development of integrated pathways and care packages
Requirements

To be successful in this role, you will need:

  • Qualified Nurse with evidence of continuing professional development
  • Experience working as a Band 6 in an acute specialism or Band 5 with long nursing experience
  • Ability to write reports and use IT
  • Teaching and presentation skills
