Job Summary

We are seeking a highly skilled and dedicated Care Experienced Young People Support Specialist to join our team in Barnsley. As a Personal Advisor, you will work closely with care experienced young people aged 16-25, providing practical support and guidance to help them navigate their transition to adulthood.

Key Responsibilities:
  • Provide individualized support to care experienced young people, addressing their unique needs and challenges.
  • Develop and implement personalized pathway plans, setting achievable goals and objectives.
  • Collaborate with local authorities, schools, and other stakeholders to ensure seamless support and services.
  • Conduct regular assessments and reviews to monitor progress and adjust support plans as needed.
  • Build strong relationships with young people, their families, and other professionals to ensure a comprehensive support network.
Requirements:
  • Excellent communication and interpersonal skills, with the ability to build rapport with young people and their families.
  • Strong assessment and analytical skills, with the ability to identify and address complex needs.
  • Ability to work independently and as part of a team, with a flexible approach to meet the needs of the service.
  • Valid DBS disclosure and ability to travel across the Barnsley borough.
