About the Role

We are seeking a highly skilled and experienced Advanced Complex Care Practitioner to join our Virtual Frailty Ward team. As an integral member of our multidisciplinary team, you will provide high-quality, person-centred care to patients in their own homes, working closely with community nursing, therapies, and other healthcare professionals to deliver individualised care plans.

Key Responsibilities
  • Conduct robust clinical triage to identify appropriate referrals and divert patients from hospital admission where possible.
  • Perform advanced holistic clinical assessments, develop person-centred care plans, and coordinate the integrated multidisciplinary team to support admission avoidance.
  • Work collaboratively with partners in health, social care, and the voluntary sector to ensure seamless care coordination and patient navigation.
  • Support patients and their families to navigate the healthcare system, promoting successful outcomes and empowering them to take control of their care.
  • Utilise technology to enhance patient care, where appropriate, and ensure safe and effective care delivery in the community setting.
