Job Summary:

The Clinical Care Coordinator will be responsible for supporting complex care packages within clients' own homes, ensuring high standards of care and safety are consistently achieved. This role involves working collaboratively with the Complex Care Manager and other stakeholders to coordinate the set-up of new client packages and provide clinical training to support workers.

Key Responsibilities:

  • To ensure effective communication between clients, families, and healthcare professionals.
  • To maintain accurate records of client care plans and progress.
  • To identify opportunities for professional development and contribute to the ongoing improvement of services.
  • To work effectively in a team environment, providing guidance and support as needed.

Requirements:

  • Demonstrated experience in managing complex care packages.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Possession of a valid UK driving license and access to a reliable vehicle.
