We are seeking a skilled Long Term Condition Nurse to join our team at Cumbria Health. As a key member of our primary care team, you will play a vital role in delivering high-quality care to patients with long-term conditions.
About the RoleThe successful candidate will have experience in managing patients with long-term conditions, including asthma, COPD, diabetes, and chronic heart disease. You will work independently and as part of a team to provide specialist nursing care, develop and implement care plans, and evaluate patient outcomes.
Key Responsibilities- Deliver high-quality care to patients with long-term conditions, including respiratory, diabetes, and cardiovascular conditions.
- Develop and implement individualized care plans, taking into account patient needs, preferences, and goals.
- Work collaboratively with the multidisciplinary team to ensure seamless care coordination and communication.
- Conduct routine assessments, evaluations, and investigations to inform care decisions.
- Provide education and support to patients and their families to promote self-management and empowerment.
- Participate in clinical governance, audit, and risk assessment to ensure high-quality care and patient safety.
- Current NMC registration and 1st Level registration.
- Relevant long-term condition management qualification or experience.
- Evidence of recent Continual Professional Development.
- Ability to work autonomously and as part of a team.
- Excellent communication and consultation skills.
- Basic computer skills.
