Key Responsibilities:

  1. Deliver statutory assessments and support planning for young people with Global Learning Disability and SEND.
  2. Work closely with the Preparing for Adulthood team and Transitions to ensure seamless transitions from children's to adult services.
  3. Maintain accurate records and reports as required.
