About the Role:

We are seeking an experienced Registered Nurse to join our team as a Dialysis Care Coordinator. The ideal candidate will have experience working in renal dialysis and be passionate about delivering high-quality patient care.

The successful candidate will be responsible for coordinating patient care activities, developing individualized care plans, and working collaboratively with other healthcare professionals to ensure seamless care delivery.

Key Responsibilities:

  • Coordinate patient care activities, including scheduling appointments and arranging transportation
  • Develop individualized care plans, taking into account patient preferences and goals
  • Work collaboratively with other healthcare professionals to ensure seamless care delivery

Requirements:

To be successful in this role, you will need:

  • Current registration with the NMC as a Registered Nurse (General/Learning Disability/Mental Health)
  • Experience working in renal dialysis or a related field
  • Strong clinical skills and knowledge of renal disease management
  • Excellent communication and interpersonal skills
