Job Title: Short Breaks Practitioner

Spencer Clarke Group is seeking a highly skilled Short Breaks Practitioner to join our team in North London.

About the Role:

We are looking for a dedicated professional to lead the coordination of short breaks and social care packages for children and young people with disabilities.

Key Responsibilities:
  • Lead Professional and Care Coordinator: Act as the lead professional and care coordinator for a caseload of disabled children with complex needs.
  • Short Breaks Assessments and Reviews: Conduct person-centred short breaks assessments and reviews of social care packages.
  • Collaboration and Coordination: Collaborate with internal teams and external agencies to deliver holistic, coordinated support for children and families.
  • Advice and Guidance: Provide advice to families about services and benefits, supporting them in decision-making processes.
  • Reporting and Presentations: Prepare reports and recommendations for resource allocation and present them at Panels.
  • Case Management: Maintain accurate case files, records, and documentation in line with departmental practice.
About You:

We are looking for a candidate with:

  • Experience Working with Children with Disabilities: Experience working with children with disabilities and their families.
  • Knowledge of Relevant Legislation: Strong knowledge of relevant legislation, including the Children Act 1989, Children and Families Act 2014, and SEND framework.
  • Assessment and Care Planning: Experience in conducting assessments of individual needs, creating care plans, and reviewing those plans.
  • Family Support: Experience supporting families in decision-making and navigating available services and benefits.

  • Competitive Salary: £20+ph
  • Contract Type: 3-6 months minimum, with a high likelihood of extension
  • Working Hours: Monday to Friday, 37 hours per week
