We are seeking a dedicated and compassionate Reablement Support Worker to join our team at Home Support Matters in Norfolk. As a Reablement Support Worker, you will play a crucial role in supporting individuals to regain independence following a period of illness, injury, or hospital admission.

The ideal candidate will have excellent communication skills, be able to work flexibly, and have access to their own transport with valid business insurance. You will work closely with individuals to set goals, monitor progress, and report achievements to ensure they receive the best possible support.

Key Responsibilities:

  • Provide a responsive and flexible service tailored to each individual's needs.
  • Focus on the individual's strengths to promote independence and well-being.
  • Support individuals in rebuilding confidence after illness, health deterioration, or injury.
  • Encourage the development of self-care skills, such as washing, dressing, cooking, and mobility.
  • Help individuals regain or retain skills, aiming for minimal or no support in the future.
  • Prevent hospital readmission or early transition to a care home.
  • Encourage social reconnection to reduce isolation.

Working Hours: Block shifts are available, running from 07:00-14:30 and 16:00-21:00, with the option to sign up for as many shifts as you like. The service operates 7 days a week.
