We are seeking a compassionate and motivated Reablement Support Worker to join our team at Home Support Matters in Norfolk. The successful candidate will be responsible for providing a responsive and flexible service to individuals in Great Yarmouth, Northern Villages, and Central Norwich, who require support to regain independence following a period of illness, injury, or hospital admission.
Key Responsibilities:- Provide tailored support to individuals, focusing on their strengths to promote independence and well-being.
- Support individuals in rebuilding confidence and developing self-care skills, such as washing, dressing, cooking, and mobility.
- Help individuals regain or retain skills, aiming for minimal or no support in the future.
- Prevent hospital readmission or early transition to a care home.
- Encourage social reconnection to reduce isolation.
- Work closely with individuals to set goals, monitor progress, and report achievements to the Field Care Supervisor.
- Excellent communication and observational skills.
- Ability to encourage and motivate individuals in a positive manner.
- Strong organisational skills and ability to manage multiple tasks.
- A collaborative team player with the ability to work independently.
- Experience in health, social care, or a similar role is desirable but not essential, as full training will be provided.
