Job Description

We are seeking a skilled Domiciliary Care Team Lead to join our Hounslow branch, responsible for overseeing care call management, staff development, and client care planning.

  • Salary: $45,000 - $55,000 per annum.
  • Industry: Healthcare and Social Care

As a Care Coordination specialist, you will be part of a dynamic team that prioritizes excellence in care delivery and client satisfaction.

Key Responsibilities
  • Managing care calls, ensuring seamless service delivery and effective communication.
  • Overseeing the care team, guiding their professional growth, and contributing to a positive work environment.
  • Developing and implementing client care plans, addressing their unique needs and preferences.
  • Maintaining CQC compliance, adhering to best practices, and upholding exceptional care standards.

Requirements
  • Prior experience in care coordination or a minimum of 2 years in healthcare and social care.
