About the Job:

This is an excellent opportunity for a Community Paediatric Nurse Coordinator to provide high-quality care to children and their families, working closely with a core team of carers to create personalised care plans.

Responsibilities:
  1. Create and implement effective care plans that cater to individual clients' needs
  2. Lead and manage multi-disciplinary teams to achieve exceptional patient outcomes
  3. Develop and maintain strong relationships with clients, families, and community teams
  4. Collaborate with healthcare professionals to ensure seamless care delivery
What We Offer:
  • Up to £50,000 per annum based on experience
  • £3,000 car allowance
  • Flexible working arrangements, including 2 days remote work and 3 days community-based work
  • Monday to Friday hours with a private pension scheme
