Care Coordination Role

This is an exciting opportunity to join our team as a Care Coordinator. You will be responsible for producing and managing staff rotas, liaising with external agencies, and conducting assessments and reviews.

Responsibilities:

  • Manage on-call duties and weekend work on a rota basis.
  • Produce Staff Rotas in advance using a computer-based roster system.
  • Assign visits to care workers based on service user needs and preferences.

Requirements:

  • Experience in rota management and scheduling.
  • Excellent organisational abilities and strong communication skills.
  • A full driver's licence and access to a vehicle.

Salary: £26,000 per annum. We offer a range of benefits, including career progression opportunities, a generous bonus scheme, 24 days of holiday plus bank holidays, and free parking.
