Job Description

The Complex Patient Care Coordinator will be responsible for supporting the safe transition and mobilisation of complex care packages from hospital to home.

This role requires flexibility and adaptability, with a strong ability to work independently and as part of a team.

Key Responsibilities:
  • To maintain high standards of care and safety for existing clients, ensuring ongoing excellence in service delivery.
  • Developing and implementing individualised care plans to meet the unique needs of each client.

The successful candidate will have extensive experience working in Intensive Care Units (ICU), spinal care units, or with complex care patients, providing specialised care and managing complex cases.

Estimated Salary: £60,000 - £80,000 per annum based on 40 hours per week, with an average of 35 hours worked per week at £35 p/h nights.

Requirements:
  • A full UK driving license and access to own vehicle.
  • Extensive experience working in ICU, spinal care units, or with complex care patients.
  • Ability to provide advanced procedures and multi-disciplinary interventions.
  • Experience in providing care to both adult and paediatric patients.
