Job Overview

We are seeking an experienced and compassionate Complex Care Nurse to join our clinical team at Prestige Nursing & Care. This is a fantastic opportunity for a dedicated professional to provide high-quality care to individuals in their own homes and community settings.

About the Role:

  • To provide clinical expertise and leadership in managing a complex caseload of clients requiring support in the community
  • Support in assessing, mobilising and implementing care plans for clients to drive business growth
  • Regularly risk assess individual client needs to ensure safe and effective care delivery
  • Provide evidence-based clinical advice and guidance on care planning and delivery across the pathway, evaluating outcomes and driving quality improvement
  • Liaise with multidisciplinary teams, commissioners, case managers and clients to build and manage a strong clinical caseload
  • A role model and leader for all staff members, promoting best practice and ensuring high standards of care

Key Responsibilities:

  • Delegation of clinical healthcare tasks required to maintain a high quality of life for clients within the community
  • Maintain high standards and proactively manage quality improvement, clinical risks, safety concerns and complaints related to client caseload
  • Drive best practice to ensure high-quality care, promoting compliance with internal policies and national standards
  • Identify, develop and maintain new business streams to support the growth of clinical services

Requirements:

  • Registered Nurse with active NMC PIN
  • Minimum 2 years of clinical nursing experience, including community or hospital settings
  • Proven management of clinical risk management and individualised care planning
  • Experience in safeguarding adults and children
  • Effective communication skills - verbal and written
  • Ability to work independently and as part of a team, self-motivated and proactive
