About the Role

This is an exciting opportunity for a dedicated professional to make a significant contribution to the continued development of high-quality local services and capable environments in South Staffordshire for people who have a learning disability. As a Band 6 Clinical Nurse Practitioner (RNLD), you will work collaboratively with your IST MDT colleagues to support a defined, shared caseload of adults who may be at risk of placement breakdown, including individuals in transition from children's services or specialist inpatient services.

The role involves utilising flexible, creative, multidisciplinary approaches to deliver specialist clinical training and updates, regular supervision, and professional leadership. You will also undertake a lead practitioner role to coordinate assessment and interventions throughout the IST clinical pathway.
