Job Description:

We are seeking an experienced Clinical Nurse Specialist to join our client's vibrant community healthcare team in Brighton. This role offers a fantastic opportunity to deliver high-quality care and contribute to the development of innovative practice initiatives.

Key Responsibilities:

  1. Develop and implement individualized care plans for patients, incorporating their unique needs and preferences.
  2. Collaborate effectively with GPs, healthcare assistants, and other team members to ensure comprehensive care.
  3. Maintain accurate patient records, ensuring compliance with clinical guidelines and regulations.
  4. Support health promotion initiatives and educate patients about managing their health effectively.
  5. Participate in quality improvement projects and contribute to the development of best practices within the team.

Required Skills and Qualifications:

  • RGN with an active NMC pin.
  • Experience in a primary care or community setting is highly desirable.
  • Proficient in administering child and adult immunizations and performing cervical smear tests.
  • Knowledge of chronic disease management is beneficial.
  • Strong communication and teamwork abilities.
  • Skilled in using electronic patient management systems.
