Key Responsibilities:

  • Monitor and follow participant care plans, documenting care provided on participant flow sheets.
  • Check voicemails and report any abnormal findings or changes in participant status to the team.
  • Assist participants with daily living activities, such as toileting and bathing, as assigned.
  • Accompany participants to appointments as needed.
  • Carry out simple treatments under the direction of nursing staff.

Requirements:

  • High School graduate or equivalent required.
  • Current certification as a H.H.A. /C.N.A. from an approved program.
  • A minimum of one year experience working with a frail or elderly population.
  • Excellent customer service and communication skills.
  • Physical capability to provide ADL and mobility assistance to all participants.
