Job Overview

We are seeking a dedicated and passionate professional to support young people as they transition out of the care system and into adulthood.

This role will focus on providing guidance and support to young people, including asylum seekers, as they navigate this critical period of their lives.

Key Responsibilities:

  • Provide individualized support to young people, including those with complex needs
  • Work closely with the Leaving Care Team to ensure a smooth transition for young people
  • Develop and maintain relationships with key stakeholders, including social workers and other professionals

Requirements:

  • Experience working with vulnerable young people
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
