Advanced Physiotherapy Practitioner in Primary Care Specialist

As an Advanced Physiotherapy Practitioner in Primary Care, you will be responsible for delivering top-tier patient care standards, catering to each individual's needs for optimal health outcomes. This will involve connecting with patients via phone or video, providing necessary support and guidance, and participating in virtual rounds to stay informed and contribute to patient management discussions.
Key Responsibilities- Uphold excellence in patient care, ensuring that each patient receives the highest standard of care.
- Provide virtual patient interactions, offering support and guidance to patients as needed.
- Attend daily virtual board meetings to stay informed and contribute to patient management discussions.
- Issue prescriptions for virtual ward patients when necessary, ensuring timely and appropriate treatment.
- Provide guidance on patient care, advising the virtual ward nursing team on patients experiencing changes in clinical status.
- Embrace flexible work dynamics, adapting to varied work settings to deliver care effectively.

- Able to identify, support and develop learning interventions for both self and MDT team members.

- Continuing professional development in portfolio.
- Practice requirements/competencies for RN/AHP in line with mandatory training.
- Knowledge of trust policy and procedures.
- National strategy for Nursing and Midwifery/AHP.
- NMC/HCPC Code.
- Project development skills.
- Awareness and respect for colleagues, patients and relatives' cultural, religious and emotional needs and beliefs.
