About the Role

We are seeking a skilled and experienced individual to fill the position of Discharge Project Lead at the Shrewsbury and Telford Hospital NHS Trust.

The ideal candidate will have a strong background in project management, volunteer coordination, and healthcare services. This role is responsible for overseeing the development and implementation of the Trust's Volunteer Discharge Project plan.

The successful candidate will lead a team of volunteers to support patients during their discharge from hospital, ensuring a smooth transition and reduced risk of readmission.

Key Responsibilities:

  • Develop and implement the Volunteer Discharge Project plan
  • Recruit, manage, and train volunteers
  • Collaborate with clinical teams to ensure seamless patient care
  • Provide ongoing support and guidance to volunteers
  • Monitor and evaluate project progress

Requirements:

  • Bachelor's degree in a related field (e.g., healthcare, social work)
  • Minimum 3 years of experience in project management or volunteer coordination
  • Strong communication and leadership skills
  • Ability to work effectively in a fast-paced environment
