An exciting opportunity has arisen to join Bristol City Council Adult Social Care as a Care Support Specialist within our Swift Response Team.

About the Role

The Swift Response Team is the first point of contact in adult care following new referrals from citizens or professionals. We are a multidisciplinary team who work together to support citizens city-wide; ensuring citizens have the right professional expertise for their presenting needs at the earliest opportunity.

Key Responsibilities

  • Manage a range of cases with varying complexity; undertaking support conversations and occupational therapy assessments.
  • Work within the statutory structure of the Care Act 2014 undertaking, Care Act Assessments/Carers Assessments/Mental Capacity Assessments and Best Interest Assessments.
  • Work closely with other agencies and departments across adult care.

Working with the Team

You will work within a team of Team Manager, Senior Practitioners, Senior Occupational Therapists, Social Workers, Social Care Practitioners, Occupational Therapists, and Care Direct Advisors. You will receive guidance and support from your colleagues to further develop your skills, knowledge, and confidence in this fulfilling role alongside professional supervision.
