Job Summary

We are seeking an experienced Dementia Care Advocate Professional to join our team in Coventry and Rugby GP Alliance. This is a unique opportunity to work with people with dementia and their families, providing expert care and support.

As a key member of our team, you will provide specialist nursing assessments, develop care plans, and coordinate care transitions. You will also facilitate education and training for staff, ensuring that they have the necessary skills and knowledge to deliver excellent care.

Person Specification

We are looking for an experienced nurse with a passion for delivering high-quality care. The ideal candidate will have:

  • Post-registration clinical experience supporting people with dementia and their families
  • Experience of completing holistic nursing assessments
  • Providing bio-psychosocial interventions
  • Working as an autonomous practitioner
  • Collaborative and multi-agency working
Salary and Benefits

The salary for this role is £45,000 - £55,000 per annum, depending on experience. We offer a range of benefits, including:
