We are seeking a skilled and compassionate Registered Nurse to join our team at Community Hospice & Palliative Care. As a RN, you will provide high-quality patient care and support to individuals and their families in a home care setting.
Key Responsibilities:
- Conduct thorough assessments of patients to identify their needs and develop effective care plans.
- Provide skilled nursing interventions, including treatments and education, to promote patient comfort and well-being.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
- Document patient progress and communicate effectively with interdisciplinary teams.
- Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
- Current license as a Registered Nurse in the State of Florida.
- Minimum of two (2) years experience in acute care, oncology, or home health care setting.
- Strong teaching, communication, and assessment skills.
- Ability to work independently and as part of a multidisciplinary team.
Work Environment:
This is a home care position, requiring the use of your own vehicle and a valid State of Florida driver's license with a reliable driving record.
