Active Care Group's Home Care division is dedicated to providing safe, reliable, and outcome-driven care to all clients through our exceptional support workers, clinicians, and management team.

We are seeking experienced, highly motivated, and proactive Support Workers with an NVQ Level 3 in Health and Social Care and a full clean driving licence to work in North Wales.

About Our Clients:

We support three male clients aged between 35 and 60 years old who reside in their own homes. All clients have spinal injuries, and some may also have additional needs such as tracheostomy, PEG feeding, and/or mental health conditions.

You will be providing daily care and encouraging our clients to engage in social activities of their choice, such as meeting friends at the pub, watching movies, and visiting family.

Your Responsibilities:
  • Ensure the safety and well-being of our clients while upholding their human rights.
  • Assist clients in exercising choice and control over their lives, taking into consideration any cognitive challenges they may face.
  • Collaborate with and support families as needed.
  • Help clients maintain a satisfactory level of hygiene.
  • Support clients in accessing local amenities like shops and leisure facilities, and attending community events.
  • Adhere to the service user plan documented for each client.
  • Maintain accurate and confidential records.
  • Participate in manual handling procedures as specified for each client.
  • Uphold professional boundaries with clients and their families, adhering to guidance from the case manager.
Your Qualifications:
  • NVQ / QCF Level 3 in Health and Social Care or a willingness to pursue it.
  • A genuine passion for providing high-quality person-centered care.
  • Experience working with clients who exhibit challenging behaviors.
  • Previous experience working with individuals with spinal injuries is desirable but not essential, as comprehensive training will be provided.
  • Excellent communication and interpersonal skills.
  • A willingness to learn and develop professionally.
