**Job Summary**

We are seeking a dedicated and compassionate Support Worker to join our Young People's Service in Hounslow. As an Enhanced Support Worker, you will play a vital role in supporting young people to reach their full potential and live independently.

**Key Responsibilities**

  • Key-working a caseload of young people, undertaking initial and continuous assessments of their needs and potential risks.
  • Co-developing positive behaviour support plans with individuals to identify triggers and actions required for positive risk-taking.
  • Supporting customers to engage with agreed support programmes, claim eligible benefits, and develop life skills.
  • Motivating individuals to prepare and cook healthy meals safely, going shopping, and offering guidance on buying balanced nutritious food.
  • Enabling customers to improve their skills to manage day-to-day activities, such as making telephone calls, dealing with correspondence, attending appointments, and form filling.
  • Supporting customers to access other services, including CMHT, substance misuse agencies, and working in partnership with other agencies as part of the support planning process.
  • Developing and maintaining links with key agencies and service providers in the local community.
  • Leading on an area of specialism (ETE, health and wellbeing) and building partnerships and developing internal and external opportunities for service-users in this area.
  • Supporting customers to maintain activities, including education, employment, training, informal accreditation, and volunteering opportunities.
  • Promoting customers to develop social skills, positive and healthy relationships, and keeping safe in various settings.
  • Engaging customers to place their voice, aspirations, and interests at the heart of service delivery, promoting inclusion, equality, diversity, and community cohesion.

**Requirements**

  • An understanding of the issues facing young people.
  • An awareness and understanding of the social, emotional, and wellbeing needs and mental health in relation to young people.
  • An awareness of adolescent development and relevant theories, such as attachment theory.
  • An awareness and understanding of child development and best practice in promoting healthy living and positive parenting skills.
  • An understanding of the challenges young parents face and the skills to enable them to pursue their aspirations while parenting effectively.
  • An understanding of the difficulties care leavers face and the skills to empower them to achieve their ambitions.
  • An understanding of the difficulties NEET young people face and the skills to enable them to develop a sense of responsibility and independence in the community.
  • An understanding of the challenges young offenders face and the skills to facilitate positive behaviour change and reintegration in the community.
  • An understanding of the roles of different agencies involved in the care and support of young people with complex needs and chaotic lifestyles, Children's Social Care, CAMHS, etc.
  • An understanding of safeguarding policies and procedures.

**Requirements**

  • Up to NVQ Level 3 or equivalent or experience in the social care/charity sector.
  • Ability to adapt to challenging environments.
  • Excellent communication skills, both verbal and written.
  • Ability to plan, manage, and evaluate outcome-focused activities and support programmes.
  • Interpersonal skills to build relationships with young people in 1:1 and group settings.
  • Strong personal resilience and excellent teamwork skills.

**Desirable**

  • Understanding of the Service legal framework, including the Children Act and Homelessness legislation, and how to use it effectively.
