We are seeking a highly skilled and dedicated Nurse Practitioner in Home Care to join our team. As a Nurse Practitioner, you will be responsible for providing high-quality, holistic care to patients in their own homes. The ideal candidate will have experience working within community settings and possess excellent assessment and intervention skills.

You will work closely with our multi-disciplinary team to deliver quality outcomes and improve the patient experience. This is an excellent opportunity to make a real difference in the lives of our patients and their families.

About the Role

  • Deliver comprehensive, evidence-based care to patients in their own homes
  • Conduct thorough assessments and develop individualised care plans
  • Work collaboratively with other healthcare professionals to achieve optimal patient outcomes

What We Offer

  • A competitive salary of approximately £28,000 - £40,000 per annum (dependent on experience)

Key Skills and Qualifications

  • Thorough and up-to-date knowledge of nursing theory and best practice
  • Recent experience working within acute medical or community settings
  • Experience of patient-related care record systems

Location

West Northamptonshire, covering areas including Northampton, Daventry, Brackley, Towcester, and Brixworth.
