Job Title: Outreach Support Worker

We are seeking a dedicated and compassionate Outreach Support Worker to join our team at Empower Family Group. As a key member of our community outreach team, you will play a vital role in supporting children, families, and carers in times of need.

About the Role

This exciting opportunity involves working in a dynamic and supportive environment, providing mentor support, and helping young people and families navigate challenges. You will be required to travel to various locations, including homes, hospitals, and community centers, to provide support and guidance.

Key Responsibilities
  • Provide emotional support and guidance to children, young people, and families in crisis situations
  • Develop and maintain positive relationships with service users, carers, and other professionals
  • Conduct home visits, hospital visits, and community outreach to provide support and guidance
  • Collaborate with the team to develop and implement support plans for service users
  • Participate in training and development opportunities to enhance your skills and knowledge
Requirements
  • Previous experience in a support work role with children, young people, and families
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Valid driving license and access to a reliable vehicle
  • Enhanced DBS check and barred list checks
