Main Duties and Responsibilities
  1. Contribute to multi-agency early support assessment of children and families and take on a lead professional role where appropriate.
  2. Lead on the delivery of the Healthy Child Programme, identifying packages of care based on the Universal, Targeted and Specialist models of service delivery.
  3. Work closely with key partners to develop and implement targeted interventions to improve the health and wellbeing of children and families.
Requirements
