The Family Caregivers of British Columbia is looking for an experienced Family Caregivers Link Worker to join their team. In this role, you will deliver individualized support to address caregiver needs within social determinants of health.

Your key responsibilities will include providing empathic 1-1 emotional support, assistance with navigating the healthcare system, and connecting caregivers with community resources addressing mental health, financial support, housing, social opportunities, exercise, and nutrition.

  • Deliver individualized support to address caregiver needs within social determinants of health.
  • Provide emotional support and guidance to caregivers, helping them navigate complex healthcare systems and connect with community resources.
  • Co-create wellness plans with caregivers to address their social determinants of health.
  • Assist with problem-solving and creating action plans using a Brief Action Planning approach.

Requirements:

  • Degree in a healthcare field or related discipline.
  • 3 years of case management experience supporting individuals and/or family caregivers.
  • Experience applying motivational interviewing techniques to support others in meeting their goals.
  • Excellent interpersonal and communication skills, with a demonstrated ability to provide empathic, warm, and professional support.
  • Proficient in Microsoft Office: Word, Excel, Outlook, and online web platforms and digital tools.

Work Details:

  • Location: Victoria, BC at FCBC's offices, with the potential to also work from home (hybrid).
  • Hours: Typically working during our office hours Monday – Friday, 8:30 AM – 4:00 PM and occasionally after-hours for caregiver appointments or other FCBC activities.
  • Compensation: $28.50 per hour, commensurate with experience.
  • Position type: Full-time, permanent.
