Job Description

As a Community Occupational Therapist, you will be responsible for conducting comprehensive assessments and providing tailored interventions for a diverse caseload of patients within the community. You will develop, implement, and review care plans that aim to enhance patients' independence and improve their quality of life.

  • Conduct assessments and provide interventions for patients in the community
  • Develop and implement care plans to improve patient outcomes
  • Support patients with rehabilitation and long-term conditions

You will also provide education and support to patients, their families, and carers, helping them to manage ongoing care needs in the community. If you have prior experience working in a community setting or possess a strong understanding of community-based care, we encourage you to apply.
